Hours (PDT): Weekday office hours are from 10am-6pm. We will take our last walk-in client at 6pm and our last scheduled client at 7pm. Saturday morning appointments are available with advanced scheduling. All clients need to down load the forms labeled Patient Information, Office Policies, Financial Policies and Privacy Policy and Patient Rights. Please sign and bring all forms with you to your first appointment. All information will be kept strictly private, as required by HIPPA. Intakes are necessary to protect both you and your Massage Therapist. Please fill out all forms completely and honestly. There are infections that can be passed through open sores, topical prescription grade pain medications that can be past through direct skin contact, and medical conditions that can be worsened due to massage. For example, massage can lower blood sugar in diabetics and women in the early stages of pregancy should avoid certain techniques. There is an intake form specifically form pregnant women. If your insurance covers massage you will need a referral/prescription form signed by your doctor or chiropractor. Most have their own, but a few prefer to use a form directly from the LMP's office. There is a link below for just such an occasion. You may also need the pre-authorization form. If you choose to print and present these forms yourself it will be automatically assumed that you have printed and received the HIPPA regulations form labeled Privacy Policy and Patient Rights. |
